If you need travel insurance for remote and extreme destinations such as Antarctica you need to pay particular attention to the fine print. Adequate medical cover is probably going to be your main concern but in the case of Antarctica, medical evacuation is going to be an unlikely possibility.

Yes you can get cover for pre-existing conditions but you need to be absolutely clear about what cover is being provided and for which conditions, and get it in writing. You might approach several insurers at the same time and request a quote for the pre-existing medical conditions you want covered. If you accept that offer now you'll need to notify your insurer of any change to your medical condition before departure.
